How Do I Register?
If you live in our practice area (Whitehaven – CA28 postcode) and wish to register as a patient you can
- Collect a new patient registration form from reception.
- download and print a registration form.
- You will need to complete this by hand and bring it to the surgery.
- We do also ask for ID including proof of address to progress your registration.

Once we have received the completed form you may be contacted by telephone to schedule a new patient appointment. This is not always necessary but if we do contact you to arrange a new patient appointment this would usually involve a 10 minute appointment with a GP and a 20 minute appointment with one of our healthcare team.
What are your practice boundaries?
We provide an open list for patients living in the CA28 area.
Any patient moving to a postcode outside of CA28 will be required to register with another practice.
It is with regret that we have had to enforce this policy.
Temporary patients
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
